Information we collect
When you schedule an appointment, we collect what you provide to the scheduling service:
- Your name, email address, and phone number
- Appointment details and any answers you provide when booking
Scheduling is handled through a third-party scheduling service, which processes this information on our behalf.
We may also collect basic website analytics, only if analytics is enabled for this site. And we store campaign attribution parameters — UTM codes and ad click IDs from the link you arrived on — in your browser's local storage for up to 90 days, and pass them to the scheduler so we know which marketing efforts are working.
This website uses the Meta pixel, a small piece of code from Meta Platforms (the company behind Facebook and Instagram), to measure our ads. It tells Meta when a page on this site is viewed, when an appointment is booked, and when a phone number on the site is tapped, along with the technical details your browser sends, such as your IP address and browser type. We do not send Meta your name, email, phone number, appointment answers, or any health information. Meta may use cookies to connect these visits to your Meta account. You can control ad personalization in your Facebook or Instagram ad settings, or block third-party cookies in your browser.
We do not ask for your Medicare number, Social Security number, or health information on this website.
How we use it
We use the information we collect to:
- Schedule and hold your appointment
- Respond to your questions
- Provide insurance guidance
- Send the required Scope of Appointment form and appointment follow-ups
- Improve this website
How we share it
- We do not sell your personal information.
- We share information with service providers acting on our behalf, such as scheduling, email, analytics, and customer management tools.
- We share information with insurance companies only when you choose to apply for coverage.
- We share information as required by law.

When you schedule an appointment, you agree to be contacted about that appointment. You can opt out of marketing messages at any time; for text messages, reply STOP.
